Black mold: Teeth? No. Gums? Unlikely. Use a scrub brush and clorox (or other brand) bleach to clean and disinfect the holder (if wall mounted). Otherwise, throw away the holder and replace both it and the toothbrush. Replace your toothbrush every 3 months regardless.
