Need time to heal: Acute kidney failure involves damage to the cells lining the kidney tubules (nephrons) which take days to regrow and repair. These cells then reestablish the high osmotic gradient in the kidney pyramids that enable concentration of urine. Until the cells regrow and the osmotic gradient is reformed, urine osmolality will be low.
