You have asked this question many times. SIADH has to be ruled out when serum sodium is low and urine osmolality is high. With appropriate antidiuretic hormone function, the kidneys should be able to get rid the water, i.e. dilute the urine (osmolality of less than 100) when serum sodium is low. Medications, thyroid or adrenal problem can be the causes besides the occult source of ADH as cancer.
