To go or not go : Well first it would depend on what is going on. If you are at risk to harm yourself or someone else, or are severely impaired in your ability to care for yourself then you definitely need to seek immediate help. If however, you are seeking an eval for recommendations for illness diagnosis and medications then this can often can be done in outpt office.
