Why do i have random acne on shoulders and back?

It's you: Acne is a complex, non-lifestyle-related, chronic, serious illness and why one person gets it and another doesn't is 90% mysterious. Trunk acne is unlikely to clear without systemic treatment. Topical benzoyl peroxide 5% 2x/day is your best friend, and your physician can give you something to take for a while that will probably clear you. Best wishes.
