Perforation: When cleaning the nose with mouth closed and with pressure, this can open the eustachian tube and indeed although not often can cause fluid to go up to the ears. For the water to come our of the ear, there has to be a perforation in the tympanic membrane which may not be new since the pressure allowed water to come out but with the acute pain, have it evaluated by md or ent.
