Serous effusion: Fluid can build up behind your eardrum if you have swelling in your throat for a few days (called eustacean tube dysfunction). Negative pressure develops in middle ear, pulls eardrum tight, and wicks off fluid. It can produce muffled sounds and occasional sharp ear pains. It resolves on its own (usually).
