Septal Perforations: They can be caused by cautery, cocaine, otc nasal sprays, nasal surgery or other more unusual diseases. They tend to crust and bleed if the septal care ledge or bone is exposed. This can be easily fixed but i don't recommend trying to patch the perforation... The failure rate is high and the scars / flaps from the surgery can block the airway and it's unnecessary to close the hole.
