Brief analysis: If this were acute, i would wonder about botulism or another neurotoxin. The chronicity might implicate myasthenia gravis, but might also consider a motor neuron disease, such as progressive bulbar palsy. Might also consider a genetic nerve or muscle disorder. ?Lyme disease. This constellation of symptoms should not elude diagnosis, and suggest consultation at a major medical center, asap.
