Anesthetics: You would not want extraction without anesthetics, as it would be painful. Dentists are quite aware of this, as anyone can just guess about the pain, too. Helping patients reach full anesthesia = numbness is the first step, so that you can have the extraction without hurting. Most patients choose all 4 to be removed in one appointment, as you need to work on post-surgery home care each time.

In my opinion if: Your medical history permits, i would recommend sedation and removal of all four at one time. The procedure is quick, the medications euphoric, and when you wake up it's all done. Why suffer twice or incur double anesthesia charges? Hope this helps.

Wisdom teeth: You will not feel anything for the procedure, whether you have 1, 2 or all 4 teeth removed at the same appointment (which is preferable). You can expect some discomfort following the procedure which varies based upon your pain threshold, the expertise of the surgeon and the difficulty of the extractions. This is handled with appropriate pain medication.
