Hard to do: It would require causing extreme physical or even greater extreme psychological stress in an individual that already has underlying cardiovascular disease or severe lung disease. You could also do so by giving someone the wrong medicine or an overdose of a medicine.

You probably can't: Keep feeding them a diet high in cholesterol, buy them cigarettes, don't encourage exercise, ignore their diabetes, and keep them away from their family doctor and you may accidentally cause more than just a heat attack. :-).
