Typically yes: Cold sores are viral lesions that spontaneously heal and should not typically leave a scar. If they are traumatized or manipulated there may be scar tissue formation as the wound heals. Of course severe outbreaks are best treated with antiviral medications. Repeated trauma to the same area may over time lead to some permanent distortion and scarring.
