Not surgery.: You have a very good doctor. Usually, epidural steroid injection should be less than 3 times at the same side. If you have no spinal cord impingement signs or unstable spine column, it should not do surgery for "relief pain". Most of unnecessary surgery would induce more injury to patient and not benefit pain control.

Is your doctor a : Surgeon? If not ask him/her to refer you to a spine surgeon for a surgical evaluation. This does not mean you need surgery, but that will give you the opportunity to get your spinal pathology evaluated by a spine surgeon. Typically if you have a compressive pathology, with persisting symptoms that has failed conservative management, you can be a surgical candidate.

Not sure: I would ask your doctor that question. If you are concerned get a second opinion. You shouldn't do anything you are concerned about until you get the answer that makes sense.
