Any dose that causes: The patient to nod off, act drunk, or stop breathing is too high. When i worked in a Methadone clinic in 1980 in chicago, the average dose was about 45 mg. With the increase in heroin potency, I am seeing patients who have been or are on 200-300 mg a day.
