Muscles heal: A muscle tear will heal with new muscle tissue but the new tissue will not be exactly the same. The details are too much to write here but in summary there is a loss of elasticity. If the tear is small, the muscle function should not be affected at all. The surrounding original tissue can carry the workload and the new tissue contributes adequately. With large tears, the function may be altered.

Yes to both: Initially fibrous "scar" material will be laid down. Some "remodeling" can occur whereby the scar tissue is replaced by new muscle.
