Yes: There are a variety of other causes of granulomatous disease in the gut; half of crohn's cases don't show granulomas in the first place, and i suspect no pathologist would make the call except upon examining the clinical and imaging findings as well. The diagnosis of crohn's, and the decision how to treat, is usually made by collaboration among physicians.
