Multiple reasons: If it has been going on for 4 weeks, gkto yourdoc, not er. Symptoms suggest multiple possibilities: left lower lobe pneumonia, ruptured ovarian cyst, kidney disease. Yourdoc should be able to determine course of action based on an exam and some labs. Pending on your relationship with your primary care doc, you might want to get pelvic exam first.
