Get worked up: The imaging specialist will advise you as to whether it's probably an old granuloma, or whether it's worth getting a tissue diagnosis as a possible cancer. Here in kansas city, many (perhaps most) folks have at least one such subpleural nodules from the histoplasmosis fungi that are common in the environment. Best wishes and glad you quit smoking.

Barely visible: If it enlarges by ct then removal is an option. It is so small at 2mm it can be hard to find. Gather any old x-rays, history of trauma, pneumonia etc. It may be an old scar but needs to be followed by pulmonologist or thoracic surgeon.
