Could be "stress": If you've had a full medical evaluation for your symptoms and no anatomical problems have been discovered, great! since you acknowledge being "really stressed and anxious for 4 months, " this warrants getting help. You could begin with a psychologist to review your various stresses and ways you respond -- and possible treatments too. On your own, meditation and/or yoga help with "stress" too.
