Yes: Plain x-rays are very good at looking for fractures, however, a hairline, nondisplaced fracture could be missed on an x-ray.

Yes quite common: Non displaced linear fractures are very difficult to visualize on radiographs even when special views (obliques) are obtained as part of examination. When fracture fragments of rib are separated or offset fracture easier to detect. If fracture is in healing phase and callus or new bone is forming causing area of fracture to become larger and more obvious, fracture much easier to visualize.
