What's your bottom?: Everybody has a different bottom. Some maybe told by their doctor that there liver is " a little inflamed" from alcohol, for them that's there bottom and they stop. Others may literally lose " everything" and keep on drinking. You do get to choose how low you want your bottom. Odds are if you have a problem and keep drinking it will get lower. It is a progressive illness left untreated.

No: Unfortunately for some, they do need to hit rock bottom before they get the wake up call to stop drinking, and then, often it is loo late. That means the person may have lost hings very important to their lives, like their families, their jobs and their freedom (jail). A person can ask for help any time before hitting rock bottom. Some do. Sadly, many do not. Good question.
