mostly anovulation: It coould be nothing, could be no ovulation, endometrial polyps, thyroid gland dysfunction, less likly infection

Endometriosis Fibroi: It can be due to many causes, but common causes are 1endometriosis 2fibroids 3cervical polyps 4hormonal imbalance 5dysfunction of the ovaries there are other rare causes which are not so common.
