varies: If he has no history of injury, the most common causes are some form of imflammatory arthritis, such as seen in patients with gout for example; or an aggravation of underlying osteoarthritis; or an overuse type syndrome such as a pes anserine bursitis. A detailed exam and appropriate imaging studies can usually make the diagnosis. Thank you.

Swollen knee: Consider effusion, synovitis, injury, meniscal or ligament tear, loose body, arthritis. Check x-ray weight bearing with orthopedic surgeon.

Effusion: Water on the knee is known as an effusion. Causes include but are not limited to inflammatory issues including arthritis, synovitis, and gout/pseudo-gout. If there has been a recent injury then the fluid can be from blood accumulation from cartilage injuries, ligament injuries, fracture, or even patellar dislocation. You should see your doctor first to determine the cause.
