Occasional bright red blood after going #2 for 5 or more years. constipated and usually with pain. w/o exam dr assumes hemmoroids. is this most likely?

Most likely.: Most likely it is hemmoridalo in etiology. I would do a colonoscopy to better define the etiologym. Could also be a anal fissure. Treatment is totally different. Proctitis could also be reason for rectal bleed.

Not just hemorrhoids: Are you sure you have just hemorrhoids? Bleeding from the rectum that recurs may also indicate anal fissures, proctitis (due to infection, inflammatory bowel disease, radiation therapy, stercoral ulcers, rectal prolapse, trauma, etc.) or a bleeding source further upstream. Depending on your age, risk factors, current medication (aspirin, nsaid's, anticoagulants can cause bleeding), get evaluated.
