Expected : At 48 some menstrual irregularity is expected as one approaches menopause. Fibroids can cause cramps.

tough to say: Without an exam, it is tough to say exactly what the cause of your missed cycle was. It could be a simple "anovulatory" cycle where you just did not release an egg, it could be perimenopausal symptoms, thyroid, stress, weight gain, weight loss, nutritional issues, etc. The pain could be fibroid in nature with your history. Best to get evaluated by gyn. Best wishes.
