Need more info : Is there a stress fracture? What has the doctor or radiologist suggested the high signal in MRI is caused by? I've never heard of micro fracture surgery for the small bones of the mid foot. I'd like you to get a 3d ct scan to rule out fracture or arthritic changes or bone tumor. Try cast and crutches with bone stimulator first before surgery.

Stabilization is key: The objective of any procedure for fractures is stability with compression. External devices allow more accurate manipulation with less disruption so are theoretically more appropriate, but the objectives are the same regardless of the approach.
