Breast augmentation: Recovery from breast augmentation over the muscle is typically easier than under the muscle due to the lack of stretch of the muscle. Both techniques are safe, however there has been a report to state that a breast implant under the muscle may provide better ease of reading on mammogram when compared with over the muscle (although there have been reports that have shown no difference as well).

Over easy Under safe: There are no absolutes. All other circumstances being the same, over is a less invasive approach that is generally less painful with a faster recovery. Under tends to require release of muscles that may involve more risk in the initial pistol period but is felt to be associated with lower overall risk of hardening and improved mammographic visualization however they may move with muscle activity.

Both ways are safe: Placing implants above or under the muscle are both safe procedures. If the implants are placed above the muscle, it is less painful and the recovery is quicker. However, it is harder to mammogram the breast and the risk of capsular contracture is higher on top of the muscle. Under muscle implants generally stay softer, but the immediate recovery is usually more painful.
