Probably not: My opinion is that it should be a continuation of the surgical procedure and covered. Your medical insurance company will likely fight it under the dental exclusion. It will be a tough fight if you choose to pursue it.

Probably not: You would need dental insurance that specifically includes orthodontic treatment, and that does not have an age 18 cap. Please know that for best success you will need orthodontic treatment before and after the jaw surgery. Please get both orthodontic and surgical consultations before beginning treatment.
