Have a large orbital floor blowout with partial herniated inferior rectus muscle and extraconal fact. have double vision when looking extremely down/up. do i need surgery? can i fully recover?

Depends: How recent was the blowout. Generally surgery is done if doubling of vision occurs or the eye is too inwardly displaced (enophthalmos). You may need surgery if this was recent and should get the advice of an oculoplastic surgeon to see if you can be helped.

Not usually: Blowout fractures with entrapment do not usually resolve on their own. In fact it should be taken care of as soon as possible because the entrapment can cause a loss of blood supply to the inferior rectus and cause double vision forever. Feel better.
