I have a 3-4cm dermoid cyst on each ovary, which cause some mild pain, usually around ovulation time. at what point should i consider surgery?

I would: Consider getting this done whenever you can. A solid cyst, if it gets a little larger, can twist on itself cutting the blood supply to the ovary. It's called an ovarian torsion and it is a surgical emergency. I just wouldn't wait too long, good luck.
