Radiculopathy: Most likely a cervical radiculopathy. A neurological examination and possibly a couple of diagnostic tests will determine the cause of your symptoms (i.e. Cervical spine MRI and emg/ncs test).

Do NOT worry: A localized twitch in one area could be very benign, but could also be due to localized nerve compression or damage. ALS is widespread as it proceeds, and not uncommonly starts with fasciculations in hand and/or thigh muscles.
