Severe changes: With cysts, they make the ovary heavier and it is possible for the ovary to twist. If you have severe pain, nausea, vomiting, you might need emergency surgical care to prevent the ovaries from twisting too much, which can block the blood supply and kill off the ovary (torsion). Otherwise they can be followed or treated depending on the shape and size.
