How accurate is a skin patch test to determine contact allergies which cause itching in absence of an obvious rash?

Patch testing: Patch testing is used to identify chemical sensitivities. Contact with these allergens causes a rash at the site of contact. If you do not have a body rash (but only itching) is much less likely that you have contact allergy as the cause of your symptoms. Start by eliminating fragranced products that may contact your skin (detergent, fabric softeners, soaps, lotions).
