I would consider: Seeing a psychiatrist. Using medication to help your brain is likely indicated. Therapy is important but for moderate to severe depression, medication with psycotherapy is the most helpful. Mention sleep dysfunction, daytime energy and pain to your doc, these are all easy to treat; hang in there.

Try your church?: Often in times of crisis it is helpful to turn to your church. You are very young and need to connect with others your age. Maybe join the youth group or young adult group? Also see if there are any support groups which would cater to a person of your age.
