If not improving then need to get skin scraping and find out types of fungal infection in labs .Hair need to be cut very short for topical medication application. See plastic surgeon: Scalp hair does not grow in a scar. The scar can be due to any reason. The best option is to see a plastic surgeon nad have him/her assess whether scar revision surgery will help you.

Get scar revision: If a scar on the scalp shows no signs of hair regrowth within 4-6 months, it is unlikely that hair will ever regrow.
