Many things: Anal fissure, abscess or anal fistula, proctitis or inflammation of the rectum, or ano-rectal tumors, benign or malignant as well as hemorrhoids(external usually) can all cause pain and bleeding, to name a few. Get checked by an expert, either colo-rectal or GI doctor to get the corret diagnosis and appropriate treatment.

See your doctor: There are a variety of causes including fissures and hemorrhoids, but you need to make sure you don't have a cancerous growth. Visit your doctor and make sure you get a complete physical exam, including a rectal exam. If you are age 50 or better and you never have had a colonoscopy, you should talk with your doctor about arranging this too.
