Yeast / Antibiotics: You were on some powerful antibiotics which killed not only the bad bacteria but also killed the good bacteria (lactobacilli) allowing the yeast to overgrow causing Vaginal itching or vulvovaginal candidiasis.  The yeast were treated with Diflucan.  You can take probiotics to replace the good bacteria in your colon, thus preventing future yeast infections. Yogurt is also helpful.

Allergy: Antibiotic allergy can cause painless itchy bumps anywhere on the body.  The cause of the rash is important to determine.  I would need to know exactly hen each antibiotic was started and stopped in relation to when the rash began to determine the culprit.  The longer you were off zosyn (piperacillin and tazobactam) the less likely it would be the cause.  An antibiotic can cause a rash even for a 1-2 dys after it was stopped.

Both are reasonable: Both are reasonable. The combination gets several different bacteria...The same as zosyn (piperacillin and tazobactam) by itself.  The difference is often the cost.

Decrease effective: Alcohol with zosyn (piperacillin and tazobactam), (piperacillin and tazobactam) decreases the effectiveness of the antibiotic and increase its side effects. Most often zosyn (piperacillin and tazobactam) is given in a hospital setting, and the likelyhood of consuming alcohol is almost nil in the hospital.

The dose listed for Levaquin (levofloxacin) seems to be right, providing no kidney issues. The doses for Vancomycin and Zosyn do not seem to be right.

Yes: Zosyn (piperacillin and tazobactam) is probably the most commonly used initial antibiotic, along with vancomycin for MRSA coverage.  There are other antibiotics that can be used, and routinely these antibiotics are changed to something more specific once culture results can be accessed.
