It depends: Both zosyn (piperacillin and tazobactam) ertapenem are very powerful antibiotic which are generally very effective for diabetic foot ulcers. One must however know which organism we are dealing with in order to determine the best choice of antibiotic. Antibiotic selection should be selected based on a culture and sensitivity.

Neither: If indeed you have an infection a proper culture should be done and the choice of antibiotic based on results.

Equal: Both would be appropriate treatment. Zosyn (piperacillin and tazobactam) would cover most strains of pseudomonas and enterococcus while ertapenem would be better if esbl producing bacteria were involved. Neither drug covers MRSA so vancomycin is usually included in empirical therapy of these infection. De-escalation and definitive therapy is determined by deep tissue or wound culture results.
