Circumcision: You can use tacrolimus forever. The .1% is more effective. Best treatment is circumcision, This is routine, quick, easy. Meanwhile you should be sure there is no urine fermenting under the foreskin. Retract the skin and rinse in the shower. Always blot the last drop of urine after you urinate.

Zoon's balanitis?: Zoon's balanitis, is an autoimmune disease, and in some cases poor higene, the best is to have circumcision. topical hydrocortisone valerate 0.2% cream, and  0.1% tacrolimus ointment.
Compliance is important to avoid recurrence of symptoms. 
Sexual transition is unusual.

Inflammation with or without infection of glans penis is balanitis. Smegma does not collect on a circumcised penis.

Causes include the following:  psoriasis, eczematous dermatitis, drug eruption, bacterial, fungal, viral infection, scabies, lichen sclerosis, erythroplasia of Queyrat (type of skin cancer).  Treatment for this disorder would typically start with topical antibacterial, antifungal and topical steroid.
