Yes, lack of sleep: Can compound any issue. Please let your treating physician know about your sleep problems. In the meantime, here is information on sleep hygiene. Retire & rise same time each day. Keep bedroom dark & cool & use only for sex & sleep. Turn off tv. No naps. Exercise regularly but not in late evening. No caffeine for 6 hrs & no alcohol or tobacco w/i 2 hrs of sleep. Keep dinner moderate sized & finish.

Side effects: Other than allergy to the medication the only really serious side effect with Zoloft (sertraline) is that it can very rarely cause suicidal thoughts in some people(usually children and young adults).  Otherwise, side effects are usually relatively minor: upset stomach, decreased libido, impaired or delayed ejaculation, dry mouth, drowsiness, agitation, insomnia.  Many of the side effects go away with time.

Yes, it is Normal: Usually this dose is given to get you back into base line meaning on normal functioning , over time your doctor can lower the dosage as the symptoms show improvement. Along with other depression manage therapies, exercise, diet one can in most cases over time may not even need medication.

Unlikely: no bad ones on low dose.  The most common ones are activation, fatigue, stomach upset, decreased sexual drive.  All should be low grade at low dose if at all.

What feeling: What is the feeling you get. Some people on cortisone can have grandiose thoughts or even psychosis. If the zoloft (sertraline) is giving you that type of feeling it is indicative of what we call cycling. This often happens to people with bipolar disorder on an antidepressant without a mood stabilizer. Let the doctor who prescribed the zoloft (sertraline) know the specifics of the way it makes you feel.

: Because you take zoloft every day and ativan as needed so it does not matter the time you take ativan because you have to take it when you have anxiety, the interaction is always there because of daily zoloft dose. Risk of fall is high at you age when taking this combination and you should not drive or operate machinery after taking ativan on top of zoloft and you should not drink.

There is no one best: Dose. Each person is different. Usually start with a sub-therapeutic dose of 25mg per day (to help the brain get used to it). Then titrate upwards. Most often - a dose between 50 to 200 mg is a good one. However sometimes we need to use "off -label" dosing (higher than  fda recommendation) to get a better response. The ultimate dose depends on how your respond (efficacy and side effects).  Be well.
