26 wks pregnant. 4th child. 34 yr old. saw obgyn for prolapse symptoms. gyn said i have a low closed cervix. if gets lower i'll get pessary. i know that's not normal. can cervix/uterus go back to normal position post delivery? vaginal delivery ok?

Common after child: bearing. It can worsen, requiring surgery. At menopause, when hormone levels drop, it can become much worse. Some level of improvement can occur, after delivery, but sometimes it gets worse. Kegel's are useful now and after delivery.

had prolapse surgery 5 yrs ago and now uterus has dropped again.  surgery is more complicated now.......how effective is a pessary and how bothersome?

Effective: Pessaries are effective but do have to be removed for intercourse and for cleaning and tend to cause a discharge and odor if not cared for properly. I have several patients who remove and reinsert them themselves and others who have to come in and have me do it. I would recommend you see a urogynecolgist who specializes in these things and discuss your options.

Clomid (clomiphene): The idea of Clomid (clomiphene) is to trigger ovulation in those who do not ovulate. If you get pregnant then you won’t have period. If you ovulated but did not get pregnant then your cycle will come on time. If Clomid (clomiphene) did not “work” and you did not ovulate then the preexisting irregular menstrual cycle that triggered the use of Clomid (clomiphene) in the first place is still in effect and the cycle will be late

Bump: Don't be embarrassed. But  have it checked by your physician. It could be just irritated skin or a small ingrown hair or a cyst or viral or bacterial infection. Your doctor can plan your treatment better if she/ he can actually sees it .

Several options: You can do nothing its generally not dangerous
You can insert a pessary to lift the uterus up its kind of like a stool for the uterus
you can strengthen your pelvic muscles with kagle exercises
you can have a hysterectomy or uterine suspension surgery .

Repair: Prolapse of the uterus that interferes with sexual activity, bladder or bowel function requires treatment.  A gyn specialist can help you decide what's best for surgical repair. A pessary is an option for some patients, but not usually those sexually active.

Kegel: Kegel exercises may help prevent theprogression of early stage of uterine prolapse.  This exercise involves contracting and relaxing the muscles that surround the vagina and urethra.  It is the same muscle you would use to stop the flow of urine while you are going to the bathroom.  It is important to do this correctly because some just tighten their abdominal muscles rather than the pelvic muscle.

Gyn check: Mild prolapse does not cause severe pelvic pain. You need a full gyn evaluation to find the true cause. Women with pelvic pressure, rectal, bladder or sexual problems due to prolapse can have surgical repair or other treatment.
