what to do if i have post-herpatic neuralgia from shingles). will an employment urine drug screen be able to distinguish between the two?

Don't understand ?: There is nothing about having post-herpetic neuralgia that will cause a positive drug screen.  If you are on medication for this, such as Neurontin or Lyrica, (pregabalin) they do not generally show up on common drug screens.  If you are being prescribed opioid pain medication, they may certainly show up, but you need to show them your valid prescriptions. If you are a bus driver, still might not be ok to take

: Head to the nearest Emergency Room, where you can be catheterized to release the urine from your bladder as the first step.

Porphyria: Porphyria is a rare disorder and few physicians have significant experience treating it. There is a skin form but other types can have skin involvement also. I think your hand symptoms are caused by porphyria.  I feel you'll have to go to a university hospital to find a specialist in this disorder if you don't have one already.

? Urinary infection: Urine is at body temperature. Uti can make urine feel hot so you can have a burning sensation when you urinate.

: If you are needing to urinate more often than every 6 hours your fluid intake is more than adequate. If you drink more you will pee more, that's the way the kidneys react.

Few things: Many causes of kidney pain including infection, kidney stones, obstruction etc. call your physician today and try will order a urinalysis and culture as well as possibly an imaging study if needed.

Shingles: Shingles is reactivation of the varicella virus. You can only get it if you had chicken pox in the past.

I don't think so.: I don't think we know what precipitates " shingles". But for all adults after age 50,  i recommend the shingles vaccine ( zostavax), to protect yourself. Most insurances cover it, but even if they don't its worth to invest  $ 200.00 having a shot.

Likely to hurt: The hot water is likely to cause you pain at the site of the rash. 
Wish you good health! - Have a diet rich in fresh vegetables, fruits, whole grains, milk and milk products, nuts, beans, legumes, lentils and small amounts of lean meats. Avoid saturated fats. Drink enough water daily, so that your urine is mostly colorless. Exercise at least 150 minutes/week and increase the intensity of exercise gradually. Do not use tobacco, alcohol, weed or street drugs in any form. 
Practice safe sex, if you have sex.

YES: There is a topical medication called zostrix which contains capsaicin which may help relieve the acute pain, as well as low dose steroids which may also help to reduce post-herpetic neuralgia.  Tricyclic antidepressants, low dose anti-convulsants and drugs like Pregabalin may help alleviate post-herpetic pain.  Also consider being vaccinated with the new vaccine.
