If fresh not old: By the time you read this it will probably be too late. However a fresh septal fracture  can be reduced  within a few days of injury  with the potential for a less visible deformity. There is a risk of bleeding and hematoma and therefore this is best done under the care of a professional. If old, it is unlikely that you will be able to correct the deformity.

Nose bleed: This is likely a small vessel of the anterior portion of the nose that is "fragile" and prone to bleeding. You may want to have it looked at and cauterized so it stops bleeding. This is an easy procedure that most physicians can do, but definitely any ENT doctor.

Septal perforation : Is the medical term.  It can cause a whistling sound when breathing in through your nose and can cause frequent nose bleeds.  The bigger question is why do you have one.  Repetitive nasal trauma, chronic nasal decongestant use, drug abuse, prior surgery, wegner's disease (autoimmune), or a cancer can cause this relatively uncommon problem.  It can be repaired with little discomfort.
