Metformin: Metformin 250 mg once per day is a very very low dose.  Max recommended dose is 2500 mg/day.  Your doctor could certainly simply increase the dose.   Make sure your creatinine (kidney test) is less than 1.4.  Transient diarrhea can sometimes be an issue with metformin.  You should know your hgba1c, which is a measure of overall control.  It should be less than 7.0.

Sugars: Yes. Absolutely.  The urine glucose would only be positive with a level around 350 mg/dl or higher.  One should never follow their sugars with urine testing.

: During an acute vertiginous migraine attack, a spike in fasting glucose levels is not typically observed. In fact, a study conducted by the American Diabetes Association found that there was no significant difference in fasting glucose levels between migraine sufferers and non-migraine sufferers. Therefore, a 60mg/dl elevation to a fasting glucose level of 150mg/dl is not considered acceptable.
