Neck: Best managed initially with pt vs chiropractic care. Stretching, massage, etc. Nsaids & possible eventual cortisone injection are options.

Chemicals: From the inside of the disk, called nucleus pulposus. They include cgrp, phospholipase a2 and others which inflame nerve roots when they leak out.

Depends: You might have neck pain or maybe not.  Might be a normal finding of a disk bulge that comes and goes. Do you have radiation of pain to either arm or shoulder?  Does your shoulder or neck have pain elicited by movement of your neck?  If i had this i would want to have "kinetic mri", which is an MRI taken while sitting up.  The usual MRI takes image in an unnatural supine position on a hard slab.

Shell vs jelly: Annulus is the rings of connective tissue, like cartilage, that form the outer part of the disk.  Nucleus pulposis is the jelly filling. 
A herniated disc occurs when the jelly squirts out of the shell whereas a "protrusion" occurs when a part of the annulus is pooching out.

i was looking closer at report of an echo from 3 years ago. it says mitral annulus calcification. i am in mid 30s. should i be concerned?

MAC: What we read on echo as mitral annulus calcification can sometimes not be real. In a young person, the chances of that echo reading being due to another cause than calcification are good. You should not be worried but you should take care to eat properly, get regular exercise and take care of any cardiac risk factors which you may have. You can check with your Dr. about the reading.

: Newer techniques to repair the annulus,which is the outer layer of an intervertebral disc, are, in my opinion, still in their infancy. They don't seem to repair the nerves within the annulus and thus I'm unsure how much pain relief they achieve.  Anti-inflammatory meds, physical therapy,epidural corticosteroid injections are tried first in annulus tear  A spine clinic can discuss the other options

No explanation: Is needed.  You basically have mild arthritic changes which are insignificant and simply as a result of time and gravity.  If you have back pain it is most likely muscular because this MRI result could be found on someone who has no pain.

Cardiac surgeon: Small aortic annulus is identified by echocardiography.  Your cardiac surgeon will plan to enlarge the annulus to accommodate the largest valve practical.  Sometimes even cutting into the heart like a ross/konno is required.  All surgery has risks but your surgeon will be everything possible to help you, remember they have dedicated themselves to this job.  Hope this helps.

Aging disc: As the disc ages the outer ring of the disk, or annulus, can dry out and crack/rupture. This can, in some cases, cause the inner part of the disk (nucleus) to push through the crack and produce what we call a disc herniation. Note that these changes can occur with or without symptoms. As such, the findings on MRI have to be closely correlated with your clinical exam to see if they are important.

Based : Based on what you have copied above, you probably have mild or grade i diastolic dysfunction, or stiffening, of the left ventricle (the pumping chamber of your heart).  This can be abnormal or can actually be normal/expected, depending on your age.  Risk of diastolic dysfunction increases with high blood pressure, obesity and coronary disease, but seeing it on an echo usually doesn't require specific treatment in and of itself.  I would encourage you to discuss with your cardiologist, if you have one.
