May be caffeine: May be that you have caffeine withdrawal headaches because you drink coffee at work but not when you have a day off.

Don't overdo it: Concussion injuries recover in variable time frames.  You probably need just some more rest time.  Understanding how long it takes to develop symptoms is helpful, as you should anticipate but not push the time to headaches and take breaks frequently. You will hear that you should avoid this type of activity; however, if you can tolerate periods of time without symptoms, you are not inducing harm.

the past few days i have been getting headaches or like tension headaches every afternoon before i get off work. it gets to the point to where i feel weak and almost sick and just want to lay down and go to sleep.  i work from 7 am to 4:30 pm.

: You need to discuss with your Dr. Ask family about migraine headaches. Get BP checked. Keep headache diary and share with a Dr.

Your GP: Or PCP will be in best position to provide such a letter as knowing best your medical history. Otherwise with no documentation of a visit and for legal purposes such notes cannot be issued and relied upon.

i hit my head 4 days ago, was under the influence of alcohol, i've been going to work but i've had a headache, do i need to get a ct scan?

Not as per your : History. If you had an accident  4 days ago and since then you are able to hold a job and besides headache have no other symptoms, then most likely you continue to watch for a day or so more for headache to go away. If you develop vomiting or fever or get dizzy then it's better to go to ER right away. Similarly any weakness in any partridge body and widening headache means you need to go to ER.

Many reasons : There are many reasons for headaches. There could be a trigger from work or not, see the list i included. See your doctor. You could be having exercise induced, tension, migraine, hypertension, mood disorder, best to see a physician.

Hypertension?: Many possibilities: high post-exertional blood pressure, neck strain, low blood sugar, exertional migraine. With multiple, reasonable possibilities, only a physical examination will resolve your problem.

Dehydration: Being dehydrated is a cause of headaches.  Try drinking water throughout the day.  You should drink before you are thirsty, and if your urine is yellow, you are not drinking enough.
