: If calcium was 10.4 mg/dl in 2020 and parathyroid level was not high, you do not have primary hyperparathyroidism as the cause even this condition is the most common cause of high serum calcium in the outpatient. I assume you aren't taking any med when answering this question. You may have a condition called hypocalciuric hypercalcemia (high serum calcium due to less calcium excreted in urine).

what are the signs of a child of four years old having a sluggish thyroid. he had blood tests that revealed he had a sluggish thyroid, he is thin and very active, the
reason they did the blood work, is that they think he has ocd.

Huh?: "thin and active" does not sound like a sluggish thyroid.  Quite the opposite, hyperthyroidism.  Also, there is no blood test for obsessive compulsive disorder (ocd).  He is too young to diagnose with a psychological disorder, if that is what you mean.  Maybe a medical problem, but not what you are indicating.

ultrasound of neck revealed enlarged thyroid most likely thyroiditis. blood tests showed elevated antibodies but no over/under active thyroid. always tired, anxious depressed. is this linked?

: Could be. Thyroiditis is usually an auto-immune inflammation and there may be other auto-immunity issues. It would be prudent to see an endocrinologist as pre-emptive thyroid replacement may be a consideration.

26yo male with tsh- 87 and free t4 - 6 . tpo antibodies over 1000. doppler ultrasonography revealed increased vascularity of the thyroid gland. which dose of the levothyroxine should i start with and do i need more tests? i weigh 65kg?

He/she will not: Armour brand dessicated thyroid is no longer the first choice of real thyroidologists to treat hypothyroidism. Be weary of anyone foisting it on you. See a real thyroidologist to find out if you really need any replacement for subclinical hypothyroidism.

PTU (propylthiouracil) and liver : Drugs like ptu (propylthiouracil) and Methimazole can cause liver toxicity but hyperthyroidism can also cause elevation int the liver enzyme test you had done. If your still early in your treatment and your thyroid hormone levels (free t4) is still high it could be that. Hope this helps.

Spinal Tap: Lumbar puncture, or spinal tap, is a test of the cerebrospinal fluid.  CSF is a clear fluid similar to saline in an IV, or eye drops, that surrounds the brain, spinal cord and spinal nerves that contains small amounts of proteins and sugars.  The physician measures the pressure in the spine and collects samples of the fluid to test for diseases like meningitis, tumors, or Multiple Sclerosis.

hard and painful neck mass for over 2.5 months on left side. ultrasound revealed a "calcified " mass. and blood tests were normal. neck hurts and slightly swollen lymph nodes on left side. pain radiating from ear to collar bone. heat/ pain in ears?

: I would think the next step would be a CT scan of the neck.  Findings indicate a chronic process.  Concern would be for malignancy of the left lobe of the thyroid gland. A biopsy will most likely be recommended to obtain a tissue diagnosis.  An experienced head and neck surgeon should be involved in your care.
