Dehydration. : Well there certainly are other things this could be (which would be elucidated by physical examination and some relevant directed testing), the symptoms as described could certainly be due to dehydration.

Several causes: It is not normal. There are several causes. Please see your PCP TO find the cause. It could be hypotension or hypoglycemia among other causes

i walked outside, got lightheaded & saw spots, and i thought i was going to faint. the only thing that helped was coughing. what could be wrong?

You may: have had a transient heart rythm abnormality which your coughing slowed down...If it happens again you may want to get it checked out! A cardiologist has a number of "tools" to investigate...speak with your Primary Care Provider for a referral or more info.

Dehydration can: Cause similar symtpms.  Unlikely to be from zoloft (sertraline).

Heat of shower: And standing can cause peripheral vasodilation and increased blood flow to the lower extremities - can cause slight drop in systolic blood pressure and if your friend has a lower end BP anyhow, this can cause temporary decreased blood flow within the brain and symptoms of  light headedness, dizziness, visual disturbances, etc.

Sobriety: I appreciate that you may not like this advise, but it sure sounds like your body needs a break.  You are putting your physical and mental health at risk. If you have the disease of addiction you are also putting yourself at risk for despair or worse.

Your brain may need some days of clarity. You may need less carbon monoxide circulating in your blood stream and more oxygen. Please take a break.

For : tiredness check hemoglobin, thyroid and vitamin D levels. Dull hearing could be middle ear issues ( effusions). Spots may have many causes. FM in differential.

Need for examination: It would require a more detailed history, physical examination and may be some tests to determine the diagnosis. If you have been having excessive menstrual bleeding, anemia from the blood loss could explain these symptoms. It would be prudent to consult your doctor.

Doctor can evaluate: There are enough arm symptoms present, so one should see a primary care doctor or an orthopedic doctor to examine the arm. Most orthopedic doctors can do x-rays in their offices, so that might make it a one-stop trip. For the lightheadedness, a primary care doctor can evaluate as to why, such as anemia, low blood pressure, infection, hormonal problems, etc...
