None that I know of: Perhaps it is a test for illegal drugs, but I have not heard of a saliva test that requires fasting.

None that I know: Most salivary tests in dentistry are to measure saliva production or to look at bacterial measurements for periodontal disease.

Not necessarily: Per http://www.forensicfluids.com/oral_v_urine.htm , Delta 9 THC has been measured in oral fluid up to 72 hours after smoking.
