Could be long thoracic or spinal accessory nerve, I have seen both .See a  good interventional pain physician to rule out intercostal neuralgia, myofascial pain (serratus/ rhomboid), thoracic disc herniation, paravertbral neuralgia, or suprascapular neuralgia.
